Compartment syndrome is unfortunately initially difficult to diagnose because many other possible causes for the pain are looked at, such as a shin fracture or bone break, and the doctor will rule these out first. Acute compartment syndrome occurs when swelling and pressure in an enclosed area or compartment within the leg becomes too great. This swelling is usually caused by an injury to the leg, but anything that impedes the flow of blood for a while can potentially cause the syndrome. Because the swelling is within an enclosed area, when it becomes more than the area can handle the high pressure causes capillaries to collapse and the blood supply to the area is cut off. Cell death occurs and can result in loss of the limb, or in extreme cases, death.

The acute pain caused by compartment syndrome is more extreme than that of a fracture or a severe sprain, and is sometimes accompanied by a tingling and burning sensation. When no fractures are present and that cause for the pain has been ruled out, a physician can measure the pressure in a department with special instruments.  If compartment syndrome is indicated, then a surgical procedure called a fasciotomy must be done. This involves the surgeon slicing open the compartment and leaving it open in order the relieve the pressure. Once that's done and the pressure is normal again, a skin graft is put in place over the wound.
When these steps aren't taken quickly enough muscle and nerve damage can occur, which can lead to permanent foot drop, loss of limb or even death.
